Short Term Subacute Rehabiliation

For short-term rehabilitation, Wauconda Health Care provides personalized rehabilitation up to seven days a week. Our staff includes:  physical, occupational, speech therapists as well as a doctor board certified in physical medicine and rehabilitation, called a physiatrists.  Our physiatrist, along with our therapists provides highly individualized rehabilitation services geared to help each person gain independence.   

Some of our specialized programs include:

  • Cardiac Care Program:  designed to provide the highest level of cardiac care management in order to prevent unnecessary rehospitalization, speed recovery and provide patients and families with education and information to help manage their care.
  • Orthopedic Rehabilitation:  skilled therapists and nursing staff work closely to provide a unique orthopedic rehabilitation program, which provides unparalleled continuity of care between your surgeon and our services.
  • Rehab-to-Home:  a specially designed program just for assisted living facilities and the residents who live there, Wauconda will provide short-term rehabilitation and work with each ALF to return the individual back to their prior living setting.
  • Pre-Surgical Rehab Reservation:  If you are planning an elective surgical procedure, you can come to Wauconda, make all your own arrangements prior to surgery and reserve a room for your post-surgical care.
  • Dementia Care Rehabilitation:  an occupational therapy-based program designed to impact cognitive function through individualized treatment targeted to each patient’s abilities.  Education is also provided to family members.
  • Stroke and Neurological Rehabilitation:  Provides a targeted and organized plan to re-learn functions lost in the shortest period of time possible, we also are one of the few area faciliteis to have therapists certified in VitalStim® Therapy.
  • VitalStim® Therapy:  is a patented therapy process and equipment system developed specifically to treat dysphagia (a muscle dysfunction which affects swallowing).  It is the only Newuomuscular Electrical Stimulation (NMES) therapy and equipment system to have received clearance from the Food and Drug Administration fo rthe treatment of dysphagia
