How to Make a Referral

Wauconda accepts admissions every day of the week, twenty-four hours a day.
To make a referral for admission, contact our Director of Admissions Cheryl
Hahn at chahn@waucondacare.com or 847.526.5551, or if the referral is made
after hours, please ask to speak to the nursing supervisor.  Please have information
about insurance or third-party pay type available as well as an overview of
the clinical condition of the patient.  Let us know what type of care you
are in need of:  traditional long-term care, Alzheimer services, respite,
Hospice or short-term rehabilitation, and we will be happy to assist you.
