Long Term Care

For persons who require traditional long-term care services, Wauconda offers some of the most companionate and individualized services meeting the needs of both the resident and the family.  Our long-term care program is designed to make everyone feel at home and at ease with the care and comfort provided.  Long-term care includes services such as: 

• Active Rehab after skilled rehabilitative care or if a person does not qualified for skilled rehabilitation services, Active Rehab focusing on the promotion of a resident’s abilities combining both activities and rehabilitation into all aspects of daily life in our facility
• Assessments and Care Planning a process which involves on-going assessment of the each resident’s potential in order to maximize independence
• Individualized Plan of Care recognizes the unique capabilities, health condition, behavior and personal preferences of the individual
• Social Services deal with the social, emotional, and environmental issues which may arise can also help families with support groups and other resources
•  Nursing Services oversees medical care, working with physicians, nursing staff, families and patients to coordinate our services  
• Dietary Services promotes good health through proper diet and therapeutic use of diets to treat various conditions
