The Rehabilitation Team

Wauconda Health Care and Rehabilitation is McHenry and Lake County’s recognized leader of rehabilitation services.  We do this through our skilled team of rehabilitation specialist’s focus on patients’s ability to gain independence and get them home or to the next level of care.  Our team consists of:  

• Medical Director of Rehabilitation performs weekly rehabilitation rounds and provides consultation to patients in our rehabilitation programs. 
• Director of Nursing oversees medical portion of rehabilitative care, working with physicians, nursing staff, families and patients to coordinate each care
• Director of Therapy oversees therapy services and provides specific management and oversight to rehabilitation therapists
• Physical Therapists works to improve the patient's strength, flexibility, balance and coordination. 
• Occupational Therapists provide services to improve the patient's ability to accomplish everyday tasks associated
• Speech Pathologists A specialist who evaluates and treats people with communication and swallowing problems help people learn how to speak properly after a stroke or other condition
• Wound Care Physician performs weekly rounds to heal diabetic, surgical and other wounds through a unique and individualized treatment plan.
• Registered Dietician promotes good health through proper diet and therapeutic use of diets to treat various conditions
• Psychiatrist and Psychologist helps with emotional recovery from a medical condition
• Social Workers deal with the social, emotional, and environmental issues which may be associated with your medical condition
• Discharge Planner helps patients transition to home by identifying and setting up community resources such as home health and arranging for medical equipment
